What to capture
For each call, log the date and time, who you spoke to and their role, the direction of the contact, and what was discussed — including anything that was promised and any follow-up you're expecting. It takes a minute, and it converts a fleeting conversation into a dated, named record.

When you're actively advocating — disputing a service denial, requesting an evaluation, escalating a concern — you can track the effort itself: the organization or advocate involved, the matter at hand, and where things stand. Everything lives alongside your child's other records, so the context is never scattered.
Why the paper trail matters
If a promised service doesn't materialize, or a decision needs to be appealed, a clear log of who said what and when is your strongest asset. "You told me on March 3rd that the paraeducator request was under review" is a very different conversation than "I think someone mentioned it a while ago."
